Improving chronic disease patients experience.
As UX designer, I defined and led strategy for Clearpath Care redefinition - a mobile/desktop application that aims to improve the connection between chronic disease patients with their medical history, appointments and requirements.

Objective
The goal was to bring a fresh perspective to significantly improve the experience of an already existing project, make its experience more intuitive and user-friendly, taking in consideration that the main users of the app would be chronic disease patients.
After an initial analysis of the users, it was important to ensure that patients could easily track their health status, manage appointments, and access personalized medical resources. Additional features such as medication reminders, symptom tracking, and real-time communication with healthcare providers were also considered essential for enhancing patient engagement and well-being.
The main solution brought to Clearpath Care is translated into one single word, humanize.
With this, it was understood that the app needed a more warm and human approach that would not only inform and have the needed management features but it also required a humanization touch, the solution for it was to bring an AI agent that would simulate the patient doctor.
Collaborative development
Regular calls were held with stakeholders and Devoteam team members, fostering a continuous feedback loop.
This collaboration enabled rapid development, ensuring timely delivery under tight and rigid deadlines, while maintaining alignment with project goals.
Patient interaction
Patient interaction with medical history, doctors, and other healthcare needs was optimized.
The new interface made it easier for patients to access medical records, schedule appointments, and view test results, improving their overall experience and sense of control over their health.
Humanizing the user experience
The design process focused on moving away from the cold, impersonal feel of the current system.
The g oal was to create a more human-centered experience, acknowledging the fragile position of chronic patients and prioritizing their needs.
The solution brought warmth and empathy into the interface, making it more engaging and responsive to patient emotions.
Virtual assistant
At the core of the solution was the creation of a virtual assistant that mirrors the doctor-patient relationship.
This assistant provides questionnaires, informative messages, and alerts based on the patient’s progress and the evolution of their condition. The goal was to offer patients timely and relevant information, supporting their journey towards better health management.
A direct messaging system between the patient and doctor powered by artificial intelligence allows patients to interact intuitively, receiving personalized instructions and alerts.
By simulating real-time communication, the AI improves engagement and makes the app feel more responsive and human-centered, supporting patients through seamless and accessible guidance.
Humanization
The guiding principle behind the solution was the humanization of the application.
Every design choice was made to ensure that the app felt more like a supportive companion than a clinical tool, helping patients feel cared for and understood throughout their medical journey.
Design
A warm color palette was chosen, departing from the previously designed scheme which had colder feeling.
This choice was made to complement the humanistic approach of the solution, making the interface feel more inviting and empathetic for patients, reflecting the core values of warmth and care.
